Changes in the valve-pressure setting have occurred after exposure of programmable valves to an MRI magnetic field . Before performing MRI on a patient with a programmable shunt, the radiologist must confirm that the shunt is resistant to reprogramming at the magnetic field strength of the scanner.

Programmable cerebrospinal shunts are a type of ventriculoperitoneal shunt that can be set to different CSF pressure settings. They are of particular value in normal pressure hydrocephalus and in pediatric patients.. The 2010 AJNR article by Lollis et al. 1 provides radiographic features (with reference images) of the settings of five common valves (Codman Hakim Programmable Valve, Medtronic MRI-induced programmable valve setting alteration. The patient with a percutaneously programmable cerebrospinal fluid shunt, presented acutely obtunded after undergoing MRI at 1.5T the previous day. The pressure settings were not checked following MRI. However, programmable shunt valves are adjusted with an external magnet.
Radiologic Identification of VP-Shunt valves and adjustment. The verification of shunt adjustment (so-called opening pressure) and also the primary identification of the implanted valve type can be a Problem in daily practice: Shunt pass is not always at hand and documents from external hospitals can be missing. A shunt is a completely inter- nalized system, as opposed to an external ventricular drain in which a ventricular catheter drains to a collection system at the bedside. The catheter components of a shunt are made from Si- lastic(DowCorning,Midland,Michigan),aformofrubbertub- ing resistant to breakdown in the body.

Ventriculoperitoneal (VP) shunts are a device used to shunt cerebrospinal fluid in the treatment of hydrocephalus. As the name suggests, a catheter is placed with its tip in the ventricle. The external portion of the catheter is connected to a valve that regulates the flow of CSF based on a preset pressure. Lollis SS, Mamourian AC, Vaccaro TJ, Duhaime A-C. Programmable CSF Shunt Valves: Radiographic Identification and Interpretation.
Reading Skull Films for Shunt Valve Settings The Codman Hakim valve is a ventricular shunt valve which can be programmed to open at various CSF fluid pressure. As with previous ventricular shunts, this valve is surgically implanted over the skull near the burr hole for the catheter.
2017-11-15 ‘From the Department of Radiology’, Division of Pediatric Radiology, Loma Linda University Medical Center and Children’s Hospital, 1 1234 Anderson St. Loma Linda, CA 92354. Presented as a 2019-06-07 The programmable CSF shunt valve has become an important tool in hydrocephalus treatment, particularly in the NPH population and in pediatric patients with complex hydrocephalus. The purpose of this study is to provide a single reference for the identification of programmable shunt valves and the interpretation of programmable shunt valve settings. In response to these findings, the ventricular shunt valve setting was lowered to 30 mm H 2 O to reduce resistance to CSF drainage. The cervical tourniquet was reapplied to increase ICP so that the ventricles would shrink. Within hours, the diplopia had disappeared and the DMS had largely resolved.
The basic building block of most shunt valves is a differential pressure “check valve” mechanism. The basic design of John Holter continues in some form more than half a century after its development.

Whenever a patient who has a shunt undergoes an MRI valve is programmable, in which case it may need to be reset. Radiologic Identification of VP-Shunt valves and adjustment .. Investigative Radiology: July 2013 - Volume 48 - Issue 7 - p 543-547 Results. Shunt valve opening pressure settings and MR-ICP were positively correlated models of cerebrospinal fluid shunt valves (CSF-SVs) from their appearance in (ASTM)14 and require monitoring and readjusting valve setting after the MRI Jun 29, 2015 A CSF shunt is composed of a proximal catheter, reservoir, valve and distal Programmable shunts allow for the setting of a specific pressure In addition to overall clinical exam and picture, radiographic imaging p valve setting and for further assessment in cases of assumed malfunction.
It is recommended to record the valve setting in the patient’s record and I.D. wallet card. Disinfect the tool kit components.
